Establishes a special fund in the State Treasury called the "Community Schools Planning and Implementation Grant Fund" to provide financial assistance to public elementary and secondary schools for community school programs and social services.

Defines "community school" as a public school providing integrated student supports (medical, dental, mental health services), expanded learning opportunities (before/after-school, summer programs, arts, tutoring), family and community engagement, and collaborative leadership practices.

Requires grant recipients to provide a community school director, establish leadership teams, implement at least two integrated student support services, offer expanded learning opportunities, and implement at least two family and community engagement strategies.

Provides maximum grant amounts: $75,000 for one-year planning grants, $250,000 annually for three-year implementation grants, and $50,000 annually for up to three years in sustainability grants for schools demonstrating success.

Becomes effective July 1, 2019; funding is contingent on legislative appropriations and will be proportionally reduced among all eligible schools if insufficient funding exists.
