Requires parents or legal guardians to sign an acknowledgment on school registration forms recognizing the importance of medical and dental physicals for enrolling children in Mississippi public schools and charter schools.

Mandates the acknowledgment statement be signed at the start of each academic school year, with specific language: "By checking this box, I am acknowledging that it is highly encouraged and important that my child, whom I am registering for enrollment in this school/school district, has a medical and dental physical."

Requires schools to communicate the acknowledgment requirement to parents or legal guardians of children who transfer between school districts if the previous school failed to obtain the required signature.

Amends Section 37-15-11 to specify that parents or legal guardians appearing for enrollment must comply with the medical and dental physical acknowledgment requirement.

Takes effect July 1, 2020.
