Directs the Governor and Division of Medicaid to negotiate with the Centers for Medicare and Medicaid Services (CMS) for a Section 1115 waiver to expand Medicaid coverage in Mississippi with healthcare delivery and payment model reform under a program called "One Mississippi."

Establishes Regional Care Organizations (RCOs) as provider-led and/or insurer-led managed care systems that must include a minimum of 75% of hospitals, primary care providers, federally qualified health centers, and community mental health centers within their defined regions.

Makes individuals ages 19-64 with household income at or below 138% of the federal poverty level eligible for coverage under the plan, with members required to opt-in and pay an annual $25 membership fee if their income exceeds 50% of the federal poverty level.

Allows RCOs to choose between two shared savings models: an upside-only sharing model (2%-3.9% minimum savings rate) or a two-sided risk model (2% minimum savings rate with both shared savings and losses).

Adds a new eligibility category (28) to Mississippi Code Section 43-13-115 for individuals eligible under the Section 1115 waiver, effective July 1, 2020.
