Enacts "Cole's Law" to prohibit discrimination against individuals with disabilities in organ transplant evaluation, placement, and coverage decisions based solely on disability status.

Requires covered entities (healthcare providers, transplant centers, and matching entities) to provide reasonable modifications, auxiliary aids, and services to enable qualified recipients with disabilities to access transplantation-related services unless doing so would fundamentally alter the nature of services.

Permits healthcare providers to consider a disability medically significant only after individualized evaluation by a physician, and prohibits denying eligibility based on inability to independently comply with post-transplant requirements if adequate support systems are available.

Establishes civil litigation remedies in circuit or county court with priority docket status and expedited review, allowing injunctive relief and facility modifications but excluding compensatory or punitive damages.

Requires health insurance issuers to provide coverage for anatomical gifts, organ transplants, and related treatment without denying coverage solely based on disability or reducing benefits for services performed pursuant to the law.
