Health care practitioners must wear a photo identification name tag during all patient encounters displaying a recent photograph, the practitioner's name, and the type of license held (e.g., physician, psychologist, nurse practitioner, podiatrist).

The name tag must be of sufficient size and worn in a conspicuous manner so it is visible and apparent to all current and prospective patients.

Health care practitioners must display in their office a clearly visible writing identifying the type of license they hold, of sufficient size to be visible to all current and prospective patients.

Practitioners who practice in multiple offices must comply with these requirements in each practice setting.

Health care practitioners not engaged in direct patient care in non-patient care settings are exempt from these identification requirements.

The act takes effect July 1, 2021.
