Creates a special "Community Schools Planning and Implementation Grant Fund" in the State Treasury to provide financial assistance to public elementary and secondary schools for planning and implementing community schools programs.

Defines "community school" as a public school providing: integrated student supports (medical, dental, mental health services); expanded learning opportunities (before/after-school, summer programs); active family and community engagement; and collaborative leadership structures.

Establishes maximum grant amounts: $75,000 one-time planning grant per school, up to $250,000 annually for three years for implementation, and $50,000 annually for up to three years for sustainability grants for schools demonstrating success.

Requires eligible schools to implement at least two integrated student supports and two family engagement strategies, establish a community school director position and leadership teams, and avoid duplicating existing programs.

Fund consists of legislative appropriations, interest accrual, donations, and other sources; funding is contingent on legislative appropriation with proportional allocation if insufficient funds exist for all eligible schools.
