Prohibits medical professionals from performing surgeries (including castration, hysterectomy, mastectomy) and administering puberty blockers or cross-sex hormones to minors to facilitate gender transition, with violations resulting in license revocation and fines up to $1,000 per occurrence.

Protects parental rights to withhold consent for gender-related medical treatments, mental healthcare, and counseling services for their children; requires government agents to notify parents immediately in writing if a minor exhibits symptoms of gender dysphoria.

Prohibits state agencies and political subdivisions from penalizing or taking adverse action against individuals who provide counsel or communication consistent with conscience or religious belief, regardless of whether it is called therapy.

Allows private lawsuits by harmed individuals against medical professionals, government agents, and entities for violations, with a two-year statute of limitations and minors able to sue until age 41; prevailing parties entitled to compensatory damages, attorney fees, and other relief.

Exempts minors with medically verifiable genetic disorders of sexual development from prohibitions; preempts local ordinances that restrict mental healthcare professionals' conduct and judgment.
