Health care practitioners must wear photo identification name tags during all patient encounters, displaying a recent photo, employee name, and license type (e.g., "physician," "nurse practitioner," "podiatrist")

Name tags must be of sufficient size and worn conspicuously to be visible to patients

Health care practitioners in collaborative practices who are not medical doctors or doctors of osteopathic medicine must conspicuously post the name of their supervising physician in their office

Practitioners must display a written notice in their office clearly identifying their license type, visible to all current and prospective patients
