Grants fund transitional assistance including market studies, telemedicine technology, health information exchange platforms, electronic health record optimization, broadband improvements, and population health management implementation

Applicants must demonstrate active community involvement in combating COVID-19 through testing, vaccination, or antibody treatment

Maximum grant award is $250,000 per business entity, with subsidiaries and affiliated health systems limited to one application under the parent entity; funded through the Coronavirus State Fiscal Recovery Fund
