Allows parents or guardians to exempt children from vaccination requirements for schools and postsecondary institutions by submitting a notarized letter or affidavit documenting which vaccinations have and have not been administered based on religious, philosophical, or conscientious beliefs.

Requires exemption affidavits to specify reasons for the exemption, document that parents were informed of vaccination risks and benefits, and include statements that unimmunized children may be excluded from school during disease outbreaks.

Establishes that exemption forms for conscientious beliefs must be obtained from the State Department of Health or county health department, expire annually, and must be renewed each school calendar year.

Authorizes exemptions from immunization requirements at licensed child care facilities when parents or guardians provide an affidavit stating the immunization conflicts with their religious, philosophical, or conscientious beliefs.

Takes effect July 1, 2023.
