Establishes parental liberty to direct upbringing, education, health care, and mental health of children as a fundamental right that cannot be substantially burdened by state or governmental entities without a compelling governmental interest of the highest order and least restrictive means.

Grants parents exclusive rights including directing child's education (public, private, religious, or homeschool), accessing school and medical records, consenting to all medical and mental health decisions, and consenting to biometric scans and DNA records.

Requires school districts to develop policies promoting parental involvement including procedures for parents to review educational materials, withdraw children from objectionable instruction (particularly sexual content and gender-related topics), and learn about clubs and extracurricular activities.

Mandates parental consent before any person, entity, or state-supported institution performs surgical procedures, physical examinations, prescribes medication, or provides mental health evaluation or treatment on a child, with exceptions only for medical emergencies.

Allows parents to bring lawsuits for violations of the Act and recover declaratory relief, injunctive relief, compensatory damages, attorney's fees, and costs; waives sovereign and governmental immunities to the extent of liability created by the Act.
