Creates new Section 93-17-206 allowing adoptees age 21 and older to request an uncertified copy of their original birth certificate from the Bureau of Vital Records, regardless of any parental affidavits prohibiting disclosure.

Requires the Bureau of Vital Records to prepare a contact preference form that birth parents may file indicating whether they want to be contacted by the adoptee, prefer contact through an intermediary only, or prefer not to be contacted.

Mandates the Bureau release any filed contact preference form to the adoptee when issuing a copy of the original birth certificate under Section 93-17-206, notwithstanding any prohibition affidavit.

Amends Section 93-17-205 to require counsel for adoptive parents to provide birth parent contact preference forms to the Bureau as part of centralized adoption records for adoptions finalized after July 1, 2005.

Takes effect July 1, 2025.
