Directs the Governor and Division of Medicaid to negotiate with the Centers for Medicare and Medicaid Services (CMS) for a Section 1115 waiver to expand Medicaid coverage to adults with incomes up to 138% of the Federal Poverty Level (FPL).

Newly eligible adults will receive private market-based health coverage through at least two Qualified Health Plans (QHPs), with the state paying premiums directly to insurers.

Establishes the Mississippi Healthy Living Account requiring enrollees with incomes between 50-138% FPL to make income-based contributions to health savings accounts, with cost-sharing capped at 2% of income for those below 100% FPL and 5% for those at 100-138% FPL.

Includes cost-sharing reductions available through participating in healthy behavior activities and guarantees enrollees cannot lose eligibility or be denied services for failure to pay copays or premiums.

Plan automatically terminates if the federal contribution rate falls below 90%; amends Mississippi Code Section 43-13-115 to establish a new eligibility category for individuals covered under the Section 1115 waiver.
