Eligibility requires Mississippi residency, a qualifying state ID, and a severe condition preventing home departure due to mobility limitations, required medical equipment, or terminal illness confirmed by physician statement.

Applicants must provide completed application, proof of residency, proof of identity, and a physician's statement specifying the medical condition and confirming homebound status with provider credentials.

Department authorized to promulgate rules for application procedures, fee waivers, and data security; Legislature must appropriate funds; department must submit annual reports on participants served, average wait times, and program challenges.
