Requires the Secretary of State to provide forms for candidates to certify their identifying information is correct and that they meet all qualifications for the office they seek.

Disqualifies candidates who voted outside the boundaries of the district they seek to represent during the period when they were required to reside within those boundaries, with an exception for voters moved by redistricting.

Establishes a judicial review procedure as the sole method for candidates to appeal an election commission's decision to exclude them from the ballot, with circuit court review, potential Supreme Court appeal, and $300 cost bonds required.

Prohibits persons removed from office for failing to meet qualifications from nominating a replacement candidate to fill the vacancy.

Applies the same voting boundary and certification requirements to special election candidates and independent candidates as to primary election candidates.
