Requires suspension of on-premises alcoholic beverage permits if two deaths from violent causes occur at the premises or adjacent property (including parking lots) within 365 days of each other, during operating hours or within one hour of closing.

Mandates revocation of the permit if a third such death occurs within 365 days of the permit's reinstatement following suspension.

Prohibits any new on-premises retailer's permit for the revoked premises for one year from revocation date, regardless of new ownership.

Permanently bars any person or entity whose permit was revoked under these provisions from obtaining an on-premises retailer's permit at any location in the future.

Requires violent death determinations to be based on police report details and applies to all establishments selling beer, light wines, or light spirit products for on-premises consumption.
