Requires pharmacy benefit managers (PBMs) and pharmacy services administrative organizations (PSAOs) to obtain annual licenses from the State Board of Pharmacy

Prohibits spread pricing by PBMs, PSAOs, carriers, and health plans, and bars PBMs from reimbursing Mississippi pharmacies less than they reimburse their own affiliates for the same drug

Establishes administrative appeal procedures allowing pharmacies to challenge reimbursements below acquisition cost, with PBMs required to respond within 30 days and adjust payments if appeals are upheld

Prohibits retaliation against pharmacists, pharmacies, or employees for exercising rights under the act or initiating legal or regulatory actions

Creates the Mississippi Independent Pharmacist Reimbursement Assistance Grant Program to provide financial assistance to eligible independent community pharmacies in the state
