Amends Medicaid eligibility criteria to update income thresholds, including raising the poverty level limit for children ages 6-19 to 133% and for pregnant women, infants, and children ages 1-7 to 143% of the federal poverty level.

Extends foster care Medicaid eligibility from age 21 to age 26, including children who aged out of foster care in other states, and adds men of reproductive age to the family planning program.

Authorizes the Division of Medicaid to implement quality or value-based payment components for nursing facilities, reduce home leave payments from 42 to 21 days per year, and reimburse pediatricians for primary care services at 100% of Medicare rates.

Establishes reimbursement for ambulance transportation providers that provide assessment, triage, or treatment services effective July 1, 2027, and prohibits Medicaid coverage for gender transition procedures.

Allows hospital assessment increases exceeding $3,750,000 per quarter to maximize federal funds for new hospital programs, and establishes a Medicaid Advisory Committee with members serving until July 1, 2029.
