Schools (excluding postsecondary) must file annual immunization reports to the state Department of Public Health and Human Services by December 1 each year, with local health department reporting requirement removed

Annual reports must include total enrollment, number of students with vaccine exemptions, number of fully vaccinated students, and number of students with conditional enrollment

Reports may only contain deidentified or aggregate data, prohibiting disclosure of individually identifiable student information

Student immunization records and exemption information are explicitly classified as education records protected under the Family Educational Rights and Privacy Act (FERPA)
