Requires all small group, large group, and individual health insurance policies in Montana to cover infertility diagnosis and treatment, including in vitro fertilization (IVF)

Establishes a lifetime coverage minimum of $40,000 for fertilization services per covered individual

Defines infertility as failure to establish or carry pregnancy after 12 months of unprotected intercourse for women under 35, or 6 months for women 35 and older, or based on physician findings or life-threatening genetic disease

Prohibits insurers from imposing different restrictions, deductibles, copayments, or waiting periods on fertility treatments than those applied to other medical services

Applies to all policies issued, delivered, amended, or renewed on or after October 1, 2025, and extends requirements to state employee and Montana university system group benefit plans
