Medical facilities must submit annual reports by February 28 to the Department of Public Health and Human Services on infants born alive following an abortion or attempted abortion, including gestational age, medical actions taken, outcomes (survival/death), and medical conditions

Facilities must file reports even if zero infants were born alive during the reporting period

Civil fines of up to $500 may be imposed for reports submitted more than 30 days late, with additional $500 fines for each subsequent 30-day period of non-compliance; the department may seek an injunction if reports are over 1 year late

The department must publish an aggregate annual report by June 30 on abortions resulting in live births, with protections to prevent identification of individuals who provided information

The department is granted rulemaking authority to implement reporting requirements and develop standardized forms for distribution to medical facilities
