Prohibits balance billing for ambulance ground transportation services, preventing ambulance providers from billing patients beyond their in-network cost-sharing amount once that amount is paid

Caps out-of-pocket costs for ambulance ground transportation at $100 maximum for both in-network and out-of-network providers, with out-of-network cost-sharing requirements matching in-network rates

Requires insurers to reimburse out-of-network ambulance services at the established local rate, or if none exists, at minimum 400% of the Medicare rate

Mandates the Commissioner of Insurance create a publicly accessible database of local ambulance rates, with ambulance services required to submit rates annually and within 5 days of any changes

Applies to health benefit plans, state employee group plans, Montana university system plans, and Medicare supplement policies, with self-funded ERISA plans able to opt in; effective January 1, 2026
