Requires all small group, large group, and individual health insurance plans in Montana to cover diagnosis and treatment for infertility, including in vitro fertilization (IVF)

Establishes a minimum lifetime coverage of $25,000 for fertilization services per covered individual

Defines infertility as failure to establish pregnancy after 12 months of unprotected intercourse for women under 35, or 6 months for women 35 and older, as well as physician findings or life-threatening genetic diseases

Prohibits insurers from imposing different exclusions, limitations, deductibles, or copayments on fertility treatments compared to other medical services

Applies to policies issued, delivered, amended, or renewed on or after October 1, 2025, and extends requirements to state employee group benefit plans and self-funded multiple employer welfare arrangements
