Establishes the North Carolina Childhood Vaccine Association as a nonprofit corporation to collect assessments from health insurers for purchasing and distributing vaccines to insured children.

Creates an 11-member Board of Directors appointed by the General Assembly (8 members), Governor (2 physicians), and Secretary of Health and Human Services, with authority to set insurer assessments and manage vaccine procurement.

Requires health insurers to pay annual assessments to the Association based on their covered lives and actual vaccine usage, with late payments subject to 150% civil penalties and interest charges.

Establishes the Childhood Immunization Account as a nonreverting restricted receipts account to receive assessment funds for purchasing, storing, distributing, and ensuring quality of vaccines recommended by the CDC's Advisory Committee on Immunization Practices.

Mandates health insurers report covered life months annually, requires health care providers to submit vaccine claims to the Board, and allows providers to select from FDA-approved vaccines available through CDC contracts while using state-supplied vaccines when possible.
