Requires the Department of Health and Human Services to provide law enforcement with personal identification information about individuals receiving DHHS services or benefits when requested in writing for active investigations or locating fugitives.

Limits disclosed information to: full name, date of birth, social security number, current and past addresses and phone numbers, names of immediate family members, and family members' contact information.

Exempts information prohibited from release by federal law from disclosure requirements.

Imposes criminal penalties: DHHS employees who fail to comply with valid requests are guilty of Class 3 misdemeanor with minimum $200 fine; law enforcement officers who request information without documented investigation or misuse obtained information are guilty of Class I felony.

Specifies that information obtained is not a public record but may be admitted as evidence in court proceedings.
