Requires physicians to notify parents or guardians within 48 hours after treating minors 12 years of age or younger for venereal disease, pregnancy, substance abuse, or emotional disturbance.

Exempts notification requirement only when the physician determines that notification would pose a serious risk to the minor's life or physical health.

Permits physicians to provide information to parents or guardians if they contact the physician about the minor's treatment.

Applies notification requirement to minors older than 12 years of age seeking treatment for the specified medical conditions.

Becomes effective upon passage into law.
