Require school principals to mandate that parents or guardians provide a certified copy of a child's birth certificate or other satisfactory evidence of birth date upon first admission to public school.

Require parents or guardians to provide a certificate of immunization as required by G.S. 130A-155 upon school admission.

Change school authorities from optional acceptance to mandatory acceptance of certified medical records from treating physicians or hospitals as secondary proof of age when birth certificates are unavailable.

Allow school authorities to accept certified birth certificates from religious institutions (churches, mosques, temples) that maintain birth records as secondary proof of age.

Make the act effective beginning with the 2011-2012 school year.
