Requires name change applicants to submit certified results of state and national criminal history record checks to the clerk of superior court.

Applicants must provide their true name, county and date of birth, parents' full names from birth certificate, desired new name, proof of county residency, and a sworn statement regarding tax and child support obligations.

Clerk of superior court must review all submitted information to determine if good and sufficient reasons exist to grant or deny the name change request.

Denied applicants may appeal to the chief resident superior court judge within 30 days, with the judge's decision being final; unsuccessful appellants must wait 12 months before reapplying.

Clerk must forward approved name change orders to the State Registrar of Vital Statistics and the Division of Criminal Information at the State Bureau of Investigation for record updates.
