Increases biennial license renewal fees from a maximum of $250.00 to $400.00.

Increases late fees for license renewal from $25.00 to $75.00.

Increases reinstatement fees from $100.00 to $200.00, duplicate license fees from $25.00 to $75.00, temporary license fees from $35.00 to $50.00, and health services provider certificate application fees from $50.00 to $100.00.

Changes the national written examination fee from "not to exceed $50.00" to a fixed fee of $50.00 in addition to vendor costs.
