Allows either parent of a stillborn child to request a certificate of birth resulting in stillbirth from the State Registrar, based on information from the fetal death report.

Certificate of birth resulting in stillbirth may omit the child's name if the fetal death report contains no name and the parent does not elect to provide one.

Certificate must clearly indicate it is not proof of a live birth.

For fetal deaths occurring before July 1, 2001, a certified copy of the fetal death report must accompany the application for the certificate.

Issuance of the certificate does not replace the requirement to file a fetal death report; effective October 1, 2011.
