Establishes licensure requirements for abortion facilities performing at least 100 abortions per calendar year, administered by the Department of Health and Human Services.

Requires facilities to submit a $750 application fee and obtain a license with initial 24-month term, renewable annually for $300; licenses are non-transferable and apply only to named premises and persons.

Mandates facilities be under physician supervision, require physicians performing procedures to have hospital privileges within 15 miles, and maintain adequate medical records; for corporations, requires a licensed physician as officer or director.

Authorizes inspections at least four times annually and allows immediate closure of facilities posing imminent serious risk to patient health or safety; establishes denial, suspension, or revocation procedures for non-compliance.

Imposes Class 1 misdemeanor penalty with fines up to $10,000 for operating without a license, and declares unlicensed facilities a public nuisance subject to abatement; effective October 1, 2011.
