Requires the Department of Health and Human Services' Divisions of Public Health and Medical Assistance, along with the State Health Plan division, to collaborate on reducing chronic disease incidence and improving care coordination.

Mandates development of wellness and prevention plans tailored to each division and identification of goals and benchmarks for chronic disease reduction.

Requires annual reports by January 1 of each odd-numbered year to legislative committees detailing financial impact of chronic conditions (cardiovascular disease, cancer, stroke, lung disease, and metabolic disease), assessment of wellness program benefits, and coordination levels among divisions.

Requires detailed action plans addressing multiple chronic conditions in individual patients, including hospital readmission reduction, transitional care planning, comprehensive medication management, and adoption of evidence-based quality measures endorsed by the National Quality Forum.

Establishes requirement for detailed budgets identifying all implementation costs and benchmarks for measuring care coordination success during the succeeding fiscal biennium.
