Establishes licensing requirements for electronic sweepstakes establishments, including a $250 application fee, operational restrictions, and prohibitions on rigged games, misleading advertising, and alcohol sales at licensed premises.

Imposes state excise taxes: $2,000 annually per establishment, $1,000 annually per device, and 4% of gross receipts from sweepstakes operations, with quarterly reporting requirements.

Authorizes counties and cities to impose additional local excise taxes up to $1,000 per establishment and $500 per device, with revenue collected and distributed by the Secretary of Revenue.

Creates civil and criminal penalties including Class 2 misdemeanor charges for operating without a license ($25,000-$100,000 fine) and civil penalties of $500-$10,000 for other violations, with license cancellation authority for non-compliance.
