Requires the Division of Medical Assistance, the Diabetes Prevention and Control Branch of the Division of Public Health, and the Division of the Department of State Treasurer responsible for the State Health Plan for Teachers and State Employees to collaborate on reducing diabetes incidence and improving diabetes care in North Carolina.

Mandates submission of a biennial report to the Joint Legislative Oversight Committee on Health and Human Services and the Fiscal Research Division by January 1 of each odd-numbered year detailing the financial impact of diabetes on agencies and the state.

Requires assessment of benefits from implemented diabetes prevention and control programs, including documentation of funding sources and amounts from the North Carolina General Assembly.

Requires description of coordination levels between the three divisions regarding activities, programs, and messaging related to diabetes prevention, treatment, and management.

Requires development of detailed action plans with specific steps to reduce diabetes impact, pre-diabetes cases, and diabetic complications, along with expected outcomes and benchmarks.
