Requires the North Carolina Department of Labor (NCDOL) to develop a standardized questionnaire and rating system to assess bidders and subcontractors on occupational safety and health performance by January 1, 2014.

Assessment criteria include safety planning, management leadership, employee participation and training, supervisor accountability, compliance record, and other occupational health and safety performance factors.

Mandates that all bidders and subcontractors complete the safety prequalification questionnaire and meet minimum safety standards set by awarding authorities to be eligible to bid on public construction contracts.

Requires bidders to provide workers' compensation coverage proof and implement a whistle-blower policy protecting employees who report workplace safety violations; bidders with false or misleading prequalification information in the previous five years are ineligible.

Allows bidders and subcontractors to apply for safety prequalification at least every six months, with awarding authorities requiring annual prequalification reviews for all participants.
