Requires the Division of Medical Assistance, Diabetes Prevention and Control Branch of the Division of Public Health, and State Health Plan Division to work collaboratively to develop plans reducing diabetes incidence, improving care, and controlling complications.

Each entity must establish measurable goals and objectives tailored to the populations they serve.

Mandates biennial reports by December 1 of even-numbered years to the Joint Legislative Oversight Committee on Health and Human Services and Fiscal Research Division.

Reports must include financial impact assessments, program effectiveness evaluations, coordination levels among entities, detailed action plans with benchmarks, and budgets identifying implementation needs and resources.

Became effective upon passage on June 26, 2013.
