Demands detailed action plans for coordinating care of multiple chronic conditions in individual patients, including measures to reduce hospital readmissions, develop transitional care plans, implement medication management, and adopt evidence-based quality standards.

Establishes requirement for departments to identify expected outcomes and benchmarks for coordinated care and estimate all costs associated with implementing the action plans.
