Establishes the North Carolina Supportive Housing Program within the Department of Health and Human Services to transition individuals with serious mental illness or serious and persistent mental illness from institutional settings to community-based supported housing using tenant-based rental assistance.

Allocates $1,745,280 for fiscal year 2013-2014 and $3,120,037 for fiscal year 2014-2015 to establish and operate the supportive housing program, plus additional funding for program administration and one-time transition stability payments not to exceed $2,000 per individual for move-in costs.

Requires Local Management Entities/Managed Care Organizations (LME/MCOs) to provide in-reach activities to inform eligible institutional residents about community housing options and to transition at least 15 eligible individuals to supported housing by June 30, 2013.

Creates the Community Living Housing Fund within the North Carolina Housing Finance Agency to increase permanent community-based housing units for individuals with severe mental illness and to increase targeted units for individuals with disabilities from 10% to 15% in housing projects.

Requires the Department of Health and Human Services to submit annual reports to the General Assembly on the number of individuals transitioned into housing slots and associated expenditures by catchment area.
