HB 13 Summary

• Requires parents or guardians to submit proof of a health assessment for each child entering public schools for the first time (kindergarten or higher grades), with assessment completed no more than 12 months before eligibility for initial school entry.

• Health assessment transmittal form must be submitted within 30 calendar days of a child's first day of attendance; failure to submit does not result in suspension, but child cannot attend school until form is provided.

• Students unable to attend due to missing health assessment form may take textbooks and digital devices home, receive missed assignments upon request, and take any quarterly or semester exams missed during the absence.

• Health assessment transmittal form is confidential and not a public record; access limited to authorized North Carolina public school administrators, teachers, and school personnel; de-identified information may be provided to Department of Health and Human Services employees.

• Departments of Health and Human Services and Public Instruction must develop a standardized health assessment transmittal form for the 2016-2017 school year and report on it to legislative oversight committees by December 1, 2015.
