Amends the North Carolina Constitution to require that all changes to the form of government and electoral district boundaries for counties, cities, and school boards must be approved by voter referendum.

Requires city governing boards to submit any changes to the city's form of government to a referendum for voter approval before implementation.

Establishes that the General Assembly must prescribe procedures for conducting referenda on these governance changes.

Repeals existing provisions allowing cities to change their form of government through ordinance adoption and removes the initiative petition process for charter amendments.

Proposes the constitutional amendment for voter approval at the May 8, 2016 statewide primary election, with implementing legislation taking effect only if voters approve the amendment.
