Establishes the Transforming Poverty Schools Pilot Program to be administered by the Department of Public Instruction, focusing on high-poverty schools to create full-service support centers for students and school communities.

Centers may be located in schools or other settings and can operate outside regular school hours; auxiliary offices may be located in local health departments to provide immunizations, vision care, hearing checks, and treatment for minor illnesses.

Schools must meet four eligibility criteria: receive an overall performance grade of F for 2013-2014, have over 85% of students qualified for free or reduced-price lunches, commit school and local resources to the program, and secure commitments from local government agencies, state agencies, and nonprofits to participate.

Requires the Department of Public Instruction to report to the Joint Legislative Education Oversight Committee by January 1, 2017, on implementation of the pilot program.
