Amends penalties for facilities licensed to care for mentally ill, developmentally disabled, substance abusers, and adult care homes by modifying violation classifications and penalty amounts.

Creates new "Type A2 Violation" category for violations resulting in substantial risk of death or serious physical harm, with penalties up to $1,000 per day for continued violations, and defines "substantial risk" as risk substantially certain to materialize without immediate action.

Allows facilities to avoid penalties for self-discovered violations that are immediately abated, corrected before Department inspection, and have reasonable preventative measures in place, provided the facility implements corrective measures.

Modifies staff training provisions to allow Department consideration of training completed after a violation occurs, in addition to ordering new training, if facility has corrected violation and maintains compliance.

Repeals the sunset date for a pilot program studying electronic supervision devices in certain facilities, making the program permanent.
