Department of Health and Human Services shall develop a streamlined single case agreement template for local management entities/managed care organizations (LME/MCOs) to use with behavioral health providers starting July 1, 2015.

Single case agreements may be used in lieu of comprehensive provider contracts when the provider is located outside the LME/MCO's service area and serves no more than two LME/MCO enrollees.

Providers using single case agreements are considered network providers under Chapter 108D of the General Statutes.

Effective July 1, 2017, Medicaid eligibility county of residence for LME/MCO enrollees is determined by: county of residence for adults 18 and older, parent or guardian's county for children, or the social services county for children in county custody.

A change of residence occurs when an individual moves to a private residence or facility where they are expected to live continuously for more than 60 days.
