Modifies the structure and appointment process for Local Management Entity/Managed Care Organization (LME/MCO) area boards in North Carolina.

Allows multicounty area authorities with a catchment population of at least 1,250,000 to adopt alternative appointment processes through resolution and Secretary approval, including delegation to intermediary bodies.

Requires at least two-thirds of constituent counties to approve alternative appointment processes in multicounty areas.

Grants area boards authority to declare vacant the office of appointed members who miss three consecutive scheduled meetings without justifiable excuse, with vacancies filled within 90 days.

Authorizes the Secretary to appoint representatives to unrepresented board categories if county boards of commissioners fail to comply with composition requirements.
