Establishes a two-tiered penalty system for facility employees who fail to report abuse, exploitation, or accidental injuries to clients in mental health, developmental disability, and substance abuse treatment facilities.

First violation of the reporting requirement is a Class 3 misdemeanor with a fine not exceeding $500.

Repeated violations are elevated to a Class 1 misdemeanor with enhanced penalties.

Prohibits threatening or harassing employees or volunteers for making reports of violations or injuries.

Effective December 1, 2015, and applies to violations occurring on or after that date.
