Discontinues the Medicaid and Health Choice Primary Care Case Management (PCCM) program effective January 1, 2016, and prohibits renewal or extension of the contract with North Carolina Community Care Networks, Inc. (NCCCN) beyond December 31, 2015.

Requires the Department of Health and Human Services to submit a Medicaid State plan amendment to the Centers for Medicare and Medicaid Services by October 1, 2015 eliminating the PCCM program, with mandatory discontinuation of all PCCM payments beginning January 1, 2016 regardless of CMS approval status.

Permits the Department to develop and utilize other managed care contracts after January 1, 2016, excluding PCCM arrangements.

Estimates annual savings of $172,000,000 in combined State and federal Medicaid and Health Choice funds for fiscal year 2016-2017, with intent to use savings to mitigate lost reimbursement to primary care providers for PCCM participation.

Removes language requiring the Department to provide North Carolina Health Choice Program services through Community Care of North Carolina at Medicaid-allowed per member, per month fees, effective January 1, 2016.
