Requires the Department of Health and Human Services to transform North Carolina's Medicaid program from fee-for-service to a capitated, risk-based managed care program by January 1, 2017.

Mandates enrollment of all eligible Medicaid beneficiaries into at least three statewide managed care organizations that assume full risk for all Medicaid benefits.

Requires the Department to submit a detailed implementation plan to the General Assembly by October 1, 2015, including proposed waivers, legislation, cost estimates, savings projections, and implementation timeline.

Directs the Department to publish a request for proposal for managed care contracts by June 1, 2016, and submit necessary State plan amendments to the Centers for Medicare and Medicaid Services.

Authorizes appropriation of funds necessary to implement the Medicaid transformation.
