Requires all health insurers providing health benefit plans to affirmatively allow insured individuals to assign their benefits to out-of-network health care providers or facilities for payment of services rendered.

Requires assignments to be in writing and acknowledged by both the insured and the provider or facility to be valid.

Limits the duration of any assignment to a maximum of one year.

Applies the requirement to limited-scope dental and vision insurance in addition to standard health benefit plans.

Becomes effective October 1, 2015, and applies to health benefit contracts issued, renewed, or amended on or after that date.
