Increases civil penalties for operating amusement devices without a certificate or in violation of regulations from $250 to $500 per day per device.

Increases civil penalties for operating after certificate refusal or revocation, failing to submit required reports, or operating without liability insurance from $500 to $1,000 per day per device.

Increases civil penalties for operating unsafe devices or while impaired from $1,000 to $2,000 per day per device.

Adds new Class E felony offense with fines up to $50,000 for willful violations that cause serious injury or death of any person.

Becomes effective December 1, 2015, and applies to violations occurring on or after that date.
