Establishes a two-year pilot program in Cumberland County developed by the Department of Health and Human Services and Department of Military and Veterans Affairs in coordination with Community Care of North Carolina and Maxim Healthcare Services to provide health care services to veterans.

Creates a health care initiative to increase veteran access to health care resources through community health worker care coordination efforts.

Establishes a workforce initiative to recruit and train unemployed and underemployed veterans as community health workers for the health care initiative.

Appropriates $400,000 in nonrecurring funds from the General Fund to the Department of Health and Human Services for fiscal year 2018-2019 to support program development and implementation.

Requires the Department of Health and Human Services to submit a comprehensive evaluation to the Joint Legislative Oversight Committee on Health and Human Services by February 1, 2021, detailing veteran access improvements, number of veterans trained as community health workers, and fund expenditures; program terminates June 30, 2020.
